Teacher Impact Grants Evaluation Form

All Impact Grant Evaluation materials are due by May 31, 2019. You can complete and submit your grant evaluation form here. Be sure to fill out each section completely. Then click the “Submit” button to send your evaluation to MEF.  Other materials, including receipts for project expenditures and any visual components of your project (photos, videos, etc.), should be submitted via email, mail or hand delivered to our office.
